What kind of throat infection is this?

It started when I was 13 or 14. At first, I noticed I had bad breath every morning when I woke up. Sometimes I would then cough up these tiny little yellow pieces of something and they smelt real bad. Year down the road, I got a mild case of geographical tongue. Now I'm 18 and in the summer, my nose got stuffed up and it's been stuffed ever since. Sometimes I can breath through it, sometimes I can't, usually in the morning after waking up. I ume it's spreading. I never went to the doctor because I was afraid of what it could be and I hoped it would go away on its on, which it clearly isn't, but it hasn't been fatal either so yeah.. I'd feel more confident about getting treatment if I knew exactly what it was.
